          Brief profile including preferred learning style, demographics (ethnicity, age, gender, free or reduced lunch status, family                          structure), likes and dislikes, etc.

          3-4 weeks of work samples with your evaluations of each

          Instructional objectives for the 3-4 week period including modifications for the child if needed

          Analysis of student learning

          Analysis of your instructional effectiveness
